Who is eligible for services from Corbrook?

Any person who has been assessed as having a developmental disability may access support from Corbrook. Documentation of assessment by a certified psychologist or physician may be requested.

Corbrook supports people who are fiscally funded through the Ministry of Children Community and Social Services (MCCSS) or people who purchase services from the organization.

Anyone inquiring about the purchase of services can contact Corbrook directly to schedule a meeting to assess suitability for services and complete the intake package.

Families requesting funding for support can connect directly with Developmental Services Ontario (DSO) at 416-925-4930 or 1-855-372-3852 who will determine eligibility. Please note that individuals must be 18 years of age and up, to be eligible for services; however, individuals may apply as early as 16 years old.

Once registration is completed with DSO, individuals will be placed on a waitlist for services. When agencies declare an opening/vacancy DSO will refer individuals based on interest and the agency’s capacity to support.

Referral Process – Person supported is referred to Corbrook through various sources (Self-Referral, Family Member, DSO, Family Support Workers, Website, Other Agencies and Schools, etc.)
